Ordering Instructions
1. Complete the "Polaris Order Form" above by selecting the model you are interested in, note any accessories you would like quoted, and fill out the bottom portion. Send this form to your Polaris rep by faxing it to 763-417-2145 or emailing to shelia.shaughnessy@polarisind.com.
2. An official quote will be returned to you.
3. Notify your Polaris contact fo make changes to your quote/order. Your official purchase order can then be submitted your Polaris rep by fax or email (see contact information above).
